  • Students will receive an email to login and "sign" the paperwork
• OHSAA form & physical are required.  ​
  • After you fill out all the forms on Final Forms, click on My Students page and for each student in band there will be a link +PPE Medical.  Click the link and print.  Fill out & sign pages 1-3, take entire print out to a doctor to review, complete & sign.  
  • RETURN completed form to Mrs. Crayton by the first day of BAND CAMP!
