The Blue Book - The official handbook of the Midview High School Instrumental Music Program

OHSAA form & physical are required  ​

After you fill out all the forms on Final Forms, click on the My Students page, and for each student in the band, there will be a link to +PPE Medical.  Click the link and print.  Fill out & sign pages 1-3, and take the entire printout to a doctor to review, complete & sign.  

All pages of the physical need to be turned in for eligibility.

RETURN the completed form to Mrs. Crayton by the first day of BAND CAMP!
